How to add a medical questionnaire / intake form

To add a medical questionnaire or intake form to a class click Yes on the option below when creating or editing a class.

A questionnaire builder will appear after clicking Edit . This contains a list of questions to choose from. Select the questions you would like to ask your clients before they join this class. If you check the first column, the client will need to answer the question before joining the class.
