<p class="wp-block-paragraph">To add a medical questionnaire or intake form to a class click Yes on the option below when creating or editing a class.</p>

<figure class="wp-block-image size-large"><img alt="ClassFit intake form builder showing medical and emergency contact questions with checkboxes selected" fetchpriority="high" decoding="async" width="1024" height="739" src="https://classfit.com/wp-content/uploads/2024/04/image-145-1024x739.png" alt="" class="wp-image-53711" srcset="https://classfit.com/wp-content/uploads/2024/04/image-145-1024x739.png 1024w, https://classfit.com/wp-content/uploads/2024/04/image-145-300x217.png 300w, https://classfit.com/wp-content/uploads/2024/04/image-145-768x554.png 768w, https://classfit.com/wp-content/uploads/2024/04/image-145-710x512.png 710w, https://classfit.com/wp-content/uploads/2024/04/image-145.png 1193w" sizes="(max-width: 1024px) 100vw, 1024px" /></figure>

<p class="wp-block-paragraph">A questionnaire builder will appear after clicking Edit. This contains a list of questions to choose from. Select the questions you would like to ask your clients before they join this class. If you check the first column, the client will need to answer the question before joining the class.</p>

<p class="wp-block-paragraph">If you would like to add your own custom questions for the medical form, you can see this tutorial&nbsp;<a href="https://classfit.com/knowledge-base/how-to-add-edit-and-delete-custom-questions-in-the-medical-form/">here</a>.</p>

<h2 class="wp-block-heading">Recurring classes</h2>

<p class="wp-block-paragraph">The intake form is customised for each class set. You do not need to set it up for every individual class, but you do need to do it for each recurring set.</p>
